Official transcripts are available at no charge. Students must use the Transcript Request Form (PDF/12K) for transcripts to be sent to themselves or directly to other institutions.

The student’s signature is required on requests for transcripts to be sent directly to the student.

All requests for transcripts must contain the following information.

1. Full name (including any former names)
2. Social Security number
3. Dates of attendance
4. Daytime phone number
5. Address to mail the official transcript
6. Signature

Request forms can be delivered in person at the Student Records Office, Room 138 of the Administration Bulding; can be sent by FAX to 903-782-0309; or can be mailed to:

Paris, TX 75460

The Student Records Office is open from 7:30 a.m. until 6 p.m. Monday through Thursday, and from 8 a.m. until noon on Friday.

Transcripts sent directly to students will be stamped “Issued To Student.”
